You became a leader. You never stopped being a clinician.

Rooted to Rise is the book written for the healthcare leader who can feel that something is structurally wrong - and has run out of patience for solutions that only work on the surface

FOUNDING READERS EDITION - PRE-PUBLICATION

The leaders we become reflect the lives we have lived.

You drove to work this morning carrying more than your role was designed to hold. You sat in a meeting where no one said what everyone was thinking. You made decisions in the space between what the data showed and what your gut already knew. And somewhere on the drive home, you wondered - again - why the same problems keep returning no matter how much effort goes in.

This is not a capacity problem. It is not a personnel problem. It is not even a culture problem.
It is an architecture problem. And it begins upstream from everything you have been trying to fix.

Rooted to Rise examines how leadership identity - formed long before any executive title was held - becomes the invisible architecture that determines whether a healthcare system produces stability or fragility under pressure. By the time the problem appears on your dashboard, the pattern producing it has been active for months.

This book shows you where it actually begins..

Healthcare Instability is not an effort failure

It is an architectural outcome

Architecture begins with identity

THE PROBLEM THIS BOOK SOLVES

The system is not underperforming because your people are not working hard enough.
It is underperforming because the architecture driving it was never deliberately designed to produce anything different.

Retention problems. Staff who have gone quiet. Communication that breaks down before it reaches you. Culture initiatives that produce temporary change and then drift back. Recurring performance issues that no policy, training programme, or leadership retreat has been able to fix. Accountability conversations that happen but don't hold.

These are not random. They are structural outcomes. And they trace back upstream to a layer that most leadership development never reaches - the identity and regulation of the leader at the top of the system.
Not as an accusation. As the most empowering truth a leader can receive.

Because if you are the upstream cause, you are also the highest-leverage point for change in your entire organization..

WHAT THIS BOOK REVEALS

Rooted to Rise introduces the Leadership Performance Cascade™ - a seven-layer framework that maps exactly how a leader's identity, regulation, and inner state shape every team and system outcome downstream.

It begins with identity. The patterns formed long before anyone had a title. How a leader responds under pressure. What their nervous system is teaching the people watching them about what is safe to say, safe to bring forward, safe to name.

Identity activates under pressure. It shapes team climate. Team climate shapes communication. Communication shapes culture. Culture shapes how work actually gets done. And how work gets done produces the performance indicators that end up on the board's agenda.

By the time the metrics shift, the pattern has been active for months.
This book shows you where it actually begins - and what becomes possible when you can finally see it clearly..

THIS BOOK IS NOT

  • A leadership tips guide
  • A motivational framework
  • A culture transformation programme
  • A burnout recovery manual
  • A collection of best practices you have already tried

THIS BOOK IS

  • A structural diagnosis of leadership architecture
  • A framework for making the invisible system visible
  • A precise model for understanding why accountability keeps failing - and where it actually needs to be redesigned
  • A tool for intervening at the layer where all downstream problems originate
  • A foundation for system stability that does not depend on one person's endurance to hold it together

The Framework: Leadership Performance Cascade™
Seven layers. One system. Everything connected.

Layer 1 - Identity Under Pressure

Who you are when things get hard. The patterns formed from lived experience become behavioural defaults under pressure - and those defaults shape system architecture before any policy does.

Layer 2 - Regulation

How you manage your internal state in real time. What your nervous system is teaching your team about what is safe to bring to you, and what is better left unsaid.

Layer 3 - Team Climate

Why some systems surface problems early and others produce silence until something breaks. What the structural difference looks like at the executive level - and what produces it.

Layer 4 - Communication Flow

Where information breaks down in healthcare hierarchies. The structural conditions that make honest escalation irrational for the people closest to the problem.

Layer 5 - Culture

How the norms of a system form and calcify. Why culture initiatives consistently underperform when the layers above them remain unexamined.

Layer 6 - Operational Behaviour

How accountability gets absorbed, deflected, or blocked before it reaches the people responsible for outcomes. Why this is a design problem, not a personnel problem.

Layer 7 - Performance

Why your retention data, patient outcomes, and board metrics reflect your architecture - and what a structural diagnosis reveals that performance management never will.

The Seven Layers - One System

Every layer is shaped by the one above it. By the time a problem appears at the bottom, it has been travelling downward for months.
Most leadership development starts at Layer 5 or below - at culture, behaviour, and performance. This book starts at Layer 1, where everything else begins.

WHO THIS BOOK IS FOR

Rooted to Rise was written for the healthcare leader who has run out of explanations for why the problems keep returning.:

  • CEOs, Executive Directors, Chief Nursing Officers, Chief Medical Officers, Vice Presidents, and Regional Health Authority Leaders who are responsible for system performance and sense that something structural is wrong
  • Senior leaders carrying recurring problems that effort, training programmes, and leadership retreats have not solved
  • The clinician who crossed into leadership and has never stopped caring about what happens at the bedside - even when the bedside is no longer where they work
  • The leader who has always known something was wrong and has never had precise enough language to name it
  • Anyone in a position of senior authority who is performing certainty every day while privately carrying everything alone

This is not a motivational book. It is not a self-help book. It is a diagnostic framework for leaders who are ready to see the architecture clearly - and redesign it deliberately.

ABOUT THE AUTHOR

Diane Gudmundson BScN MN MBA spent 25 years inside healthcare as a nurse, nurse practitioner, and the founder of her own mobile clinic. She drove the clinic herself. She absorbed the caseloads of three nurse practitioners when they left in quick succession - twelve to fourteen hour days, seven days a week, until her body made clear that what she was running was not a sustainable model.

What she eventually understood was that she had not been failing at leadership. She had been succeeding at the wrong thing - running a clinical identity inside a leadership role, and paying the full price for it.

The clinician in her had never stopped. The care had never stopped. But no one had ever told her that the person she was at the bedside was not a liability in leadership. It was her most powerful asset - and it needed a different kind of structure to work from.

That recognition became the foundation of the Leadership Performance Cascade™ and the central question of this book: why do the same problems keep returning, regardless of how hard people work?

The answer is architecture. And it begins at the identity layer.

Diane holds a Master of Nursing and an MBA with a capstone focused on healthcare leadership architecture. She was recognized as Best Healthcare Leadership Strategist in Canada, 2026 by the Evergreen Awards.

Rooted to Rise is her first book.
